PARENT/GUARDIAN PHOTO, VIDEO & MEDIA CONSENT RELEASE

Attendees: Ages 13–17

1. NOTICE OF PHOTOGRAPHY AND RECORDING

I understand that Club M.A.D. (Making A Difference) will have photography and videography taking place throughout the Launch Party.

Photographs, video footage, and audio recordings will be captured to document the event, celebrate teen participation, promote Club M.A.D., and support its nonprofit mission.

These materials may be used publicly, including on social media, websites, promotional materials, fundraising campaigns, and other organizational communications.

2. PARENT/GUARDIAN AUTHORIZATION

I confirm that I am the minor attendee's parent or legal guardian, or otherwise legally authorized to provide this consent.

By accepting this agreement, I authorize Club M.A.D., its authorized photographers, videographers, staff, volunteers, and representatives to photograph, film, record, and otherwise capture my minor child's image, likeness, voice, statements, and participation during the event.

3. AUTHORIZED USE OF MEDIA

I grant Club M.A.D. permission to use, reproduce, edit, publish, display, distribute, and share these materials for lawful organizational purposes, including:

  • Social media posts, reels, stories, and videos.

  • Club M.A.D.'s website and digital platforms.

  • Promotional materials and advertisements for future events.

  • Fundraising campaigns, donor outreach, and sponsorship presentations.

  • Newsletters, community outreach, and public relations.

  • Event recaps, organizational reports, and presentations.

  • Communications with community partners, sponsors, and supporters.

This authorization includes use in digital, printed, and other media formats.

4. PUBLIC DISTRIBUTION AND PRIVACY

I understand that authorized photographs, videos, and recordings may be published publicly and may be viewed, downloaded, shared, or redistributed by third parties.

Club M.A.D. cannot guarantee the removal of materials already published, distributed, or shared by others.

Club M.A.D. will not intentionally publish the minor's full name, personal contact information, or other sensitive identifying information in connection with event media without separate authorization.

5. COMPENSATION AND RELEASE

I grant this authorization without compensation and understand that neither the minor nor I will receive payment, royalties, or other compensation for authorized use of these materials.

To the extent permitted by applicable law, I release Club M.A.D. and its authorized representatives from claims arising solely from the authorized use of the minor's image, likeness, voice, or recordings in accordance with this agreement.

This release does not authorize unlawful, defamatory, misleading, or otherwise unauthorized uses and does not waive claims that cannot legally be released.

6. DURATION AND WITHDRAWAL

This authorization applies to media captured during the November 7, 2026 Launch Party and permits its continued use for the organizational purposes described above.

I may contact Club M.A.D. to request that future use of identifiable materials featuring my minor child be discontinued.

Club M.A.D. will make reasonable efforts to honor such requests for future publications. I understand that previously published, printed, distributed, or third-party-shared materials may not be fully removable.

7. FINAL ACKNOWLEDGMENT

By affirmatively accepting this agreement, I acknowledge that:

  • I am legally authorized to provide media consent for the minor attendee.

  • I understand that photography and videography will occur during the event.

  • I authorize Club M.A.D. to capture and use identifiable photographs, videos, audio, and likenesses of the minor for the purposes described.

  • I understand that authorized content may be published publicly, including for social media, fundraising, advertising, and event promotion.

  • I have read and voluntarily agree to this Media Consent Release.
